The Transitional Rehab Center at St. John’s United is a carefully designed 36 bed unit which is led by skilled nursing, rehab staff and care managers who assist in the transition from the hospital to home or an alternative care setting. Our staff work closely with patients and families to achieve their short-term rehabilitation goals while closely monitoring medical concerns with the assistance of 24-hour nursing staff and rounding providers. While receiving comprehensive therapy up to six days a week as appropriate and 24/7 nursing services throughout the duration of your stay, our care managers additionally provide assistance with discharge planning and collaborate closely with patients and families for additional needs.
Facility amenities include an onsite pharmacy, fee-based laundry services, private rooms with ensuite bathrooms, some patient rooms with private patios, local phone, cable and internet access, an always available menu with specials daily, including drinks and snacks, and notary services if needed. An outdoor courtyard, coffee shop, church services, activities, and a restaurant are available for use with family as appropriate.
